What are the potential consequences to a project of leaving even a single known risk unmanaged? Why are some risks not managed?

The potential consequences are an unknown (because unmeasured, because ignored) probability of the risk being fully realized.

Why are some risks not managed?

The short answer is, people think they are managing them, when in fact they’re doing something else.
What are they doing instead?

  • trying to plan around them;
  • trusting in people’s experience, skill and good intentions.

You should plan to avoid risk as much as is possible. You should trust the people you work with. But these things are not risk management. Risk management is something entirely different.
